Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor Degree in Nursing field or other relevant area of study.
  • Minimum of three (3) years of recent clinical experience in Home Infusion Nursing.
  • Ability/Experience with Inserting IVs
  • ONS/ONCC Chemotherapy/Immunotherapy Certificate Course obtained within three (3) months of hire (Agency provides paid training)
  • Competency Requirements With previous and recent IV experience within one (1) year: Demonstration with preceptor inserting peripheral line, and accessing ports, PICCs and Central Lines by end of new hire probationary period Without previous and recent IV experience within one (1) year: Will complete a vascular access device (VAD) course through HMH and follow up practicum experience in HMH outpatient infusion setting and return demonstration with preceptor in inserting peripheral line and accessing ports, PICCs and Central Lines by end of new hire probationary period
  • Successful completion of Infusion Test within probationary period with grade of 80% or higher
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms
  • Excellent written and verbal communication skills

 

Education, Knowledge, Skills and Abilities Preferred:

  • Master of Science in Nursing
  • Minimum of five (5) years of recent clinical experience in Home Infusion Nursing
  • PICC/Midline Placement Proficiency

 

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License
  • BLS AHA Basic Health Care Life Support HCP Certification

 

Licenses and Certifications Preferred:

  • Certified Registered Nurse Infusion
  • Vascular Access-Board Certified

Responsibilities

The Home Infusion Nurse Educator is a Registered Professional Nurse who is responsible for documentation review of all visit assessments as well as for continued education and development of the Infusion South and Infusion North nursing teams in order to promote documentation and educational outcomes that are consistent with the goals of the department.

 

  • Review every admission chart to determine that all pertinent information the clinicians document is appropriate and to ensure all quality indicators are completed at the initial encounter with the patient.
  • Perform chart reviews to determine if the documentation is appropriate/accurately completed per patients¿ diagnosis and Infusion therapy. Identify areas of concern through focused clinical audits. Trend data obtained from reports and review results with Managers of Clinical Practice. Report issues to the Director of Nursing.
  • Demonstrates the ability to work with team members regarding documentation corrections. Demonstrates flexibility and resourcefulness when interacting with team members while ensuring information is accurate and timely. Identify areas of improvement for team members that have been identified with inconsistencies in documentation, or documentation that does not accurately reflect the patients' condition. Collaborate with team member's Manager of Clinical Practice to develop a plan of action/follow-up.
  • Regularly shadows nurses in the field on patient visits and performs assessments, analyzes results and identifies training opportunities. This field shadowing includes providing direct patient care as needed in order to demonstrate skills for education/remediation purposes.
  • Develops and designs educational offerings based on assessments and departmental/organizational requirements.
  • Delivers education offerings and in-services and documents objectives, evaluations, and attendance.
  • Evaluates the effectiveness of educational offerings to ensure that knowledge and/or skills learned are being applied on the job and having an impact on staff performance in meeting departmental/organizational goals.
  • Promotes the professional development of staff. Plans and implements in-services based upon identified learning needs of staff and patients.
  • Participates in educational programs generated by the department.
  • Collaborates with Managers of Clinical Practice and the Director of Nursing to enhance educational support in assigned areas (Infusion South/Infusion North) based on specific needs.
  • Acts as a mentor and evaluates with input from preceptors, the clinical performance of new staff.
  • Serves as an expert clinician and role model: identifies, develops, and assists in the implementation of current practice standards.
  • Provides hands-on training as scheduled. (RE: documentation, physical assessments, clinical/case management skills.)
  • Performs direct patient care visits as necessary and directed by management to assist during periods of patient surge
  • Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.
  • Adheres to HMH Organizational competencies and standards of behavior.
  • Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs, and stands a minimum of 8 hours a day.

Starting Minimum Rate

Starting at $111,924.80 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
