Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor's Degree.
  • 6-8 years of Healthcare leadership experience.
  • Proven track record of program oversight and leadership.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.


Education, Knowledge, Skills and Abilities Preferred:

  • Proficient in Google Office Suite.
  • Ten (10)+ years of Healthcare leadership experience.

Responsibilities

Responsible for the strategy, execution and management of the HMH Network-wide Hospital From Home program which encompasses multiple HMH hospitals across the South, Central and Northern regions. Oversees the contractual relationship with Medically Home for care delivery for all patients served under the model. Serves as a liaison between Medically Home, SCP Health, population health leadership and hospital leaders for all elements of Hospital From Home. Hospital From Home Nurse Managers have a dotted line matrix reporting relationship to the Program Manager.

  • Oversight, analysis and leads continuous improvement of daily business operations to meet goals for: 
       - Volume growth through geographical, clinical use case and payor expansion
       - Expense management
       - Charge capture accuracy and Rev Cycle processes for consolidation of MRNs (escalations in care)
  • Develops and presents program clinical improvement and operational outcome reports to internal and external stakeholders including CMS and other regulatory bodies.
  • Ensure consistent and effective deployment of the Hospital From Home Operating Model in collaboration with clinical leadership and Medically Home support team to meet goals for:
       - Patient Experience
       - Patient intake optimization and clinical case use expansion
       - Patient safety and quality
       - Clinical staffing, productivity and rapid response service provider efficiency (MHG primary responsibility)
  • Ensures high reliability with patient safety and quality processes in the Hospital From Home program.  Collaborates with the clinical leadership team to review safety and quality outcomes, including near misses, develop, implement and evaluate continuous improvement plans.
  • Supports Command Center clinical leadership and serves as 24/7 Administrator on Call for escalations.
  • Seeks input on workflows/strategies and serves as change agent with:
       - Referring providers, specialists, and other downstream providers in collaboration with Medical Directors
       - Nursing, Ancillary and non-clinical leaders in collaboration with Hospital From Home Nurse Managers
  • Oversight of partnership relationship with Medically Home, fostering trust, mutual respect, and accountability for outcomes.
  • Develops strong partnership with executive sponsor(s) to secure/maintain executive level support and change advocacy.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.
  • Lifts a minimum of 5 lbs, pushes and pulls a minimum of 5 lbs, and stands a minimum of 2 hrs/day.

Starting Minimum Rate

Starting at $169,124.80 Annually

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
